Adult Homes | Care Homes | An Adult Home is established and operated for the purpose of providing long-term residential care, room, board, housekeeping, personal care and supervision to five or more adults unrelated to the operator. Adult Homes may be operated by a natural person, a partnership, a not-for-profit corporation, a public corporation or a non-publicly traded business corporation. |
Affordable Senior Housing | Affordable Senior Housing | HUD 202 Program that offers rental assistance for seniors who meet the requirements of the federal program. |
Assisted Living | Assisted Living | An Assisted Living provides Nursing Home eligible residents of Adult Homes or Enriched Housing with home health care in addition to the services routinely provided by the Adult Home or Enriched Housing operator. This program is licensed and regulated by the New York State Health Department. |
Continuing Care Retirement Community | Continuing Care Retirement Community | New York State approved Continuing Care Retirement Communities (CCRC) are housing and care complexes is restricted to seniors. They include independent living units (apartments and / or cottages), social activities, meals, supportive assistance and personal care all on one campus. Nursing Home care is also included and may be provided on or off-campus. Residents’ housing and health care are covered under a life-care contract or under a long-term housing and health care contract. The Community’s health-related and nursing care are licensed and regulated by the New York State Health Department, and the Community’s financial status and pricing structure are overseen by the New York State Insurance Department. |
Enriched Housing Programs | Care Homes | An Enriched Housing Program is established and operated for the purpose of providing long-term residential care to five or more adults, primarily senior persons 65 years of age or older, in community-integrated settings resembling independent housing units. An Enriched Housing Program provides or arranges for the provision of room, board, housekeeping, personal care and supervision. Enriched Housing Programs may be operated by a natural person, a partnership, a not-for-profit corporation, a public corporation or a non-publicly traded business corporation. |
Home Health Agencies | Home Health Agency | Home Health Agencies offer services in the homes of senior or disabled people with temporary or chronic needs who cannot perform day-to-day tasks. Services may range from nursing care to assistance in shopping, meal preparation and other household functions, as well as bathing, grooming and feeding. |
Health Care Programs | Home Health Care | As an alternative to institutional long term care, Long Term Home Health Care (LTHHC) Programs provide Nursing Home services and other services to persons in their own homes or apartments. Elderly or disabled individuals are eligible for the LTHHC Program if an assessment indicates that they are Nursing Home eligible and the services required will cost no more than a specified percentage of what those same services would cost if provided in a Nursing Home. |
Hospice | Hospice | Under Medicare, Hospice is primarily a comprehensive program of care delivered in a person’s home. Hospice care provides all the reasonable and necessary medical and support services for the management of a terminal illness. |
Licensed Home Care Services Agency | Licensed Home Care Services Agency | Offers home care services to clients who pay privately or have private insurance coverage. These agencies may also contract to provide services to Medicare/Medicaid beneficiaries whose cases are managed by another provider or entity, such as providing home health aide services to a certified home health agency patient or providing a licensed practical nurse for a Medicaid prior-approved private duty nursing shift. |
Nursing Homes | Nursing | Nursing Homes are facilities that provide skilled nursing services and chronic custodial care to elderly or disabled patients. They are licensed and regulated by the New York State Department of Health. |
Residence for Adults | Care-Homes | A Residence for Adults (Residence) is established and operated for the purpose of providing long-term residential care, room, board; housekeeping and supervision to five or more adults, unrelated to the operator. |
